Community Health Workers

Community Health Workers (CHW) help patients set goals, make care plans, and find the community resources patients need.

Home Missouri Highlands Health Care (MHHC) has several CHWs assigned to different programs and placed in different MHHC clinic locations. All with the same goal, to care for patients and our local communities in all aspects of life.

Assist patients with addressing social determinates of health needs
Specialist and/or Medical Supply Referrals
Goal Setting
Home Visits
Basic Screenings & Assessments
Health Education and/or Informal Counseling
Advocate for individual and community health needs
Outreach & Engagement
Help locate food pantries and other community resources
Medicaid Applications / Care Coordination of Medical services to Health Departments and other health care entities
Assist with obtaining the COVID-19 vaccine
